Formales of Elgin; beloved daughter of Fidelino and Susan (nee Bayona); loving sister of Kristel, Kaye (Jae) Alejo, Kevin and Kirk; dear grandaughter of Victoria and the late Bienveindo Formales and the late Cresenciano and Carmelita Bayona; proud aunt of Connor; cherished girlfriend of Ramdon Magpayo; many dear uncles, aunts, nieces and nephews.
Visitation Thursday, April 20, 3:00 - 9:00 p.m. at the Countryside Funeral Home & Crematory, 95 S. Gilbert St. (at State St.), South Elgin.
Visitation continues Friday, April 21, 9:30 until the time of mass 10:30 a.m. at St. Thomas More Church, 215 Thomas More Dr., Elgin. After mass cremation will take place at the Countryside Crematory. 847-289-8054.
